How to Make Family Status Changes – Life Event

Just follow these steps. Before you begin you may want to print out this page of instructions so that you can refer to it as you log on.

1. After reading through these instructions, click on the “Family Change” button located at Step 9.
2. You will come to the log-on screen. Enter your 9-digit employee ID (found on your pay stub, beginning with 802……, or in your benefits package on your Enrollment Statement). *
3. Enter your password. For the first time only, your password is the last 4 digits of your social security number followed by your 2-digit month (e.g. February is 02) and 2-digit day of birth. (You create a new password for future use once you logon – so you may have already done this).

4. Once logged on, click on “Employee Self Service” (a) on the left side of the screen.
Then click on “Benefits,” (b)
and then “Benefits Home” (c)


5. Under Life Events, click on the desired option (e.g. marriage, birth).


6. Read the instructions on the screen and then click on “Start.”

7. Click on the appropriate event and click “Next.”


8. Choose the appropriate date for the event (it must be a past or present date) and click “Submit”


9. Click “Yes, I want to enter information about my new dependent(s) now.”

10. Add your dependents by clicking “Add a dependent or beneficiary.” Once completed and saved, click the link at the bottom of each page until you return to the “Birth/Adoption Life Event” page. Then click “Click here to continue.”

11. Click “Next” to page through the “Benefits Summary” and “Payroll” pages, making payroll changes as needed.

12. Click on “Yes, I’d like to prepare my benefit options now.” At the next screen click on “Prepare Benefit Options” and then “Cancel” to continue.

13. Click “Click here to continue with your Life Event”

14. Click “Enroll in benefits.” Click “Select” for the appropriate event and make your changes to the appropriate benefits.

15. Don’t forget to hit “Submit” when you have completed your elections.

Family Status Change (FSC) Questions and Answers
Q: What do I do if I forget my password
A: You will be expected to re-set your password the first time you login. If you forget the password after that, please contact the Resource Center at 1-877-ICISERV. Note: you will need your 9-digit employee ID (802…….) when you call the Resource Center.

Q: What if I do not have access to a computer to enroll?
A: Your local library or school system will have access to computers for your use. If you do not have access through these means please contact the Resource Center at 1-877-ICISERV.

Q: The left menu shows the same headings as those in “Benefits Home.” Can I use those instead?
A: Yes. Both navigations will get you to the same place if they are named the same.

Q: What happens if I do not enroll within 30 days of my Family Status Change?
A: If you do not enroll within 30 days you will not be able to enroll/remove your dependents.

Q: What do I do if after completing the Family Status Change form online, it states that “My benefits could not be prepared” or there is no link for enrollment.
A: If this happens, contact the Resource Center at 1-877-ICISERV.

Q: I am changing my benefits after my Family Status Change (FSC) and the system will not allow me to change my medical and dental plans...Why?
A: During an FSC you can only make changes to plan level - not the plan itself.

Q: Why can’t I do a Family Status Change (FSC) for a date after today’s date?
A: You can only register an FSC for today’s date or prior.

Q: When can I expect my Confirmation Statement?
A: You can expect it approximately 5 to 7 business days from when you submit your benefits online.

Q: I have enrolled in all of my plans. How do I complete my enrollment?
A: Once you are satisfied with your elections, just click on the “Submit” button at the bottom of the enrollment “New Hire Enrollment” page.

Q: How do I go back to the previous screen when enrolling?
A: When enrolling it is best to go back to the “Benefits Enrollment” screen by selecting that heading on the left, and then when the new screen comes up select the benefit again.

Q: Will I be kicked out of enrollment if I wait too long without doing anything on the screen?
A: You will be logged off after 20 minutes of inactivity.

Q: Can I enroll beneficiaries in the enrollment process?
A: Yes, just click “Edit” on the Life, AD&D or Supplemental AD&D plans.

Q: Where do I enroll my dependents?
A: When enrolling in each plan you will be asked to add or enroll your dependents at that time.

Q: Why don’t the dependents I add show up in the plan to enroll?
A: Dependents need to be of an eligible age and can only be a son, daughter, spouse or domestic partner. Check your dependent to ensure this is the case.

Q: Why do I have to go through the Payroll screens prior to finishing my FSC?
A: The tool you are using wants to offer you the opportunity to change this information in the course of your FSC because these are common changes that are made.

Q: Where can I change my Lifestyle benefits (e.g., GUL, Vision, Pet Insurance, Home/Auto insurance, etc.)?
A: These plans need to be changed through Marsh. Please see your Enrollment Guide or the benefits website for more information.
