How to Make Beneficiary Additions or Changes

Just follow these steps. Before you begin you may want to print out this page of instructions so that you can refer to it as you log on.

1. After reading through these instructions, click on the “Beneficiary Change” button located at Step 11.
2. You will come to the log-on screen. Enter your 9-digit employee ID (found on your pay stub, beginning with 802……, or in your benefits package on your Enrollment Statement). *
3. Enter your password. For the first time only, your password is the last 4 digits of your social security number followed by your 2-digit month (e.g. February is 02) and 2-digit day of birth. (You create a new password for future use once you logon – so you may have already done this).

4. Once logged on, click on “Employee Self Service” (a) on the left side of the screen.
Then click on “Benefits,” (b)
and then “Benefits Home” (c)


5. Click “Benefits Elections Summary” (d) at the top of the Benefits Home screen

6. Click on one of the plans that allows beneficiaries - i.e., Life, AD&D or Supplemental AD&D - (e).


7. To add or change an available beneficiary click the “Edit” button (f) at the bottom of the screen.


8. If your beneficiary is there, allocate the percentage or amount (g) of your insurance that you wish.


9. If your beneficiary is not present,click on “Add a New Beneficiary” (h), follow the directions to add your beneficiary and return to the “Change Current Beneficiary and Allocations” page to allocate your percentage or amount to your newly added beneficiary.


10. Once you have allocated the appropriate amount or beneficiary to your beneficiary click “Save” and repeat the following instructions for the previous plans that allow beneficiaries.

Q: What do I do if I forget my password?
A: You will be expected to re-set your password the first time you login. If you forget the password after that, please contact the Resource Center at 1-877-ICISERV. Note: you will need your 9-digit employee ID (802…….) when you call the Resource Center.

Q: What if I do not have access to a computer to add/change my beneficiary?
A: If you do not have access to a computer you will need to contact the Resource Center at 1-877-ICISERV.

Q: The left menu shows the same headings as those in “Benefits Home.” Can I use those instead?
A: Yes. Both navigations will get you to the same place if they are named the same.

Q: Is there a specific time I need to add/change my beneficiary?
A: No. You may add/change your beneficiaries at any time. Just remember, if you elect a beneficiary electronically it will supersede any paper election.

Q: Will I be kicked out of the beneficiary screens if I wait too long without any activity on the system or I walk away from my desk for a period of time?
A: Yes. You will be logged off after 20 minutes of inactivity.

Beneficiary Questions and Answers
Q: Will I receive confirmation of my beneficiary add/change?
A: No. If you wish to retain documentation just print your allocation screen for your records.

Q: Once I have input all of my beneficiaries, how do I ensure my information is saved?
A: Once you are satisfied with your allocations just click on “Saved” at the bottom of the page.

Q: Can I also enroll beneficiaries during the enrollment process?
A: Yes, you can enroll beneficiaries: Just click “Edit” on the Life, AD&D or Supplemental AD&D plans.

Q: When I add my beneficiaries will they also show up as eligible dependents?
A: They will show as eligible dependents only if they are within the age limit and are listed as spouse, son, daughter or domestic partner adult.

Q: When adding my beneficiary do I need to add their social security number?
A: No. This is recommended, but not necessary. You only need to add first and last name as well as the information labeled with an asterisk.

Q: When allocating insurance amounts to my beneficiaries can I have the same person be primary and secondary?
A: No. You can only name a person as primary or beneficiary…not both.

Q: Can I add beneficiaries in the “Dependent/Beneficiary Info” page?
A: No, but you can edit the information of already existing dependents or beneficiaries.
